Your Visit
A comprehensive medical examination of your eyes takes between one hour to one-and-a-half hours.

You will receive dilating eye drops to allow a more accurate examination. These drops will cause your eyes to temporarily become more sensitive to light and may blur your vision. For your comfort when leaving the office, we recommend that you bring a pair of sunglasses. You also may prefer to have a relative or friend drive you home.

Your Appointment
To schedule an appointment at our Western Hills office, call us at (513) 389-9911 between 8 AM and 5 PM, Monday through Friday. When scheduling an appointment, please inform the scheduler for the reason for your visit and if you are being referred by another physician. This information will help us schedule the adequate amount of time for your care.

Calling Us
Please feel free to call our office if you have any questions about your condition, medication or treatment. Requests for medication refills will be taken only during regular office hours.

Our experienced staff is trained to answer most of your questions. If you require a doctor’s advice, our staff will talk with the doctor between patients and relay the answer to you. The doctor is available for emergency calls. Please be sure to leave a correct telephone number, and when and where you can be reached.

Eye Emergencies
For any eye emergency, please call our office at (513) 389-9911. If you are calling after regular business hours, you will be given our answering services phone number. They will contact Dr. Kondash for you.

Fees and Insurance
If you have medical insurance, please bring your insurance card with you. We will file all claims to your insurance company. If your insurance company requires a referral, please provide it or you will be asked to reschedule your appointment. If you are enrolled in an HMO plan, your co-payment is due at the time of service. We accept Medicare assignment.

For services Medicare does cover, it pays 80 percent the remaining 20 percent as well as any uncovered services, are the responsibility of the patient.

Insurance
If you do not have medical insurance, payment is due at the time of your office visit, We accept cash, personal checks, Visa, Mastercard and Discover, If you have difficulty paying on your account, please notify us immediately. We can assist you with a payment plan.
